Enrollment

Welcome to the Ball-Chatham School District! We're glad you're here. New students and their families are welcome at any of our schools to tour our facilities and meet our staff. When enrolling, please have the following forms ready to present to our schools:

Students will also need the following:

  • Student in Good Standing Transfer Form

  • Copy of Certified Birth Certificate

  • Test Scores (Achievement, ISAT, PARCC, IAR)

  • Transcripts from previous school

  • Copy of current physical and immunizations


Medical Information and Forms

All new students must have a current physical and proof of immunizations. They are also required to show proof of an eye exam performed by an Illinois licensed optometrist.

Requirements for New K-12 Students

All new students must have a current physical and proof of immunizations.

  • Physical Exam: Illinois law requires a current physical exam, less than one year from the 1st day of school, when entering 6th grade. Students without a current exam on file will not be allowed to attend the 1st day of school unless there is an appointment card from the doctor’s office on file with the school nurse.

  • A Dental Exam is required for Kindergarten, 2nd, 6th, and 9th grade students in Illinois. The form is due May 15 (dated within 18 months of May 15th deadline). If you are without dental insurance or cannot afford the exam you will be required to fill out the Dental Exam Waiver for their child.  The waiver form will fulfill the state requirement.

  • All out of state students moving into the district are required to show proof of an eye exam performed by an Illinois licensed optometrist.

  • Tdap: (Tetanus, Diphtheria, Pertussis) four or more doses are required with the last being a booster that received (needs "is" added) on or after the 4th birthday.

  • Hepatitis B: Three doses required. The first two doses are no less than 28 days apart and the interval between the 2nd and the 3rd doses must be at least two months apart.

  • MMR: (Measles, Mumps, Rubella) two doses are required.  The 1st dose on or after the 1st birthday and the 2nd dose no less than 28 days after the first.

  • MCV: (Meningococcal) All students over the age of 11 years old are required to be vaccinated with a single dose of a quadrivalent meningococcal vaccine.

  • OPV/IPV: (Polio) ]three or more doses are required with the last dose received on or after the 4th birthday. Any combo vaccine with IPV requires four doses.

  • Varivax: (Chickenpox) two doses required.  The 1st dose on or after the 1st birthday and the 2nd dose no less than 28 days after the first, OR PROOF OF DISEASE

  • Tdap: (Tetanus, Diphtheria, Pertussis) 6th grade requires one dose of vaccine.

Required for Kindergarten -

  • ​Complete list of all immunizations:

    • Diphtheria, Tetanus, Pertussis vaccine (DTP/DTaP)-must have received four or more doses with the last dose being a booster and having been received on or after the 4th birthday.  

    • Measles, Mumps, and Rubella vaccine (MMR)-must have received two doses with the first dose on or after the 1st birthday and the second dose no less than four weeks after the first dose. 

    • Polio vaccine (OPV/IPV)-must have received three or more doses of all IPV or all OPV vaccine or four or more doses of any combination of IPV and OPV. The last dose must have been received on or after the 4th birthday. 

    • Varicella vaccine (Chickenpox)-must have received two doses with the first dose on or after the 1st birthday and the second dose no less than four weeks after the first dose. Or provide proof of the child having had the disease. 

  • Physical Exam: Current State of Illinois school physical examination. A current physical is an exam dated less than one year from the 1st day of school

  • Eye Exam: Performed by a licensed Optometrist or Ophthalmologist.   

  • Dental Exam: Illinois law requires a current dental exam for Kindergarteners. The dental exam is to be turned in by May 15th of the current school year.  It can be dated up to 18 months of the May 15th deadline. If parents are without dental insurance or cannot afford the exam they will be required to fill out the Dental Exam Waiver for their child.  The waiver form will fulfill the state requirement.

Required for Second Grade -

  • Dental Exam: Illinois law requires a current dental exam for 2nd graders. The dental exam is to be turned in by May 15th of the current school year.  It can be dated up to 18 months of the May 15th deadline. If parents are without dental insurance or cannot afford the exam you will be required to fill out the Dental Exam Waiver form for their child.  The waiver form will fulfill the state requirement.

 

Required for Sixth Grade -

  • Complete list of all immunizations:

    • T-DAP booster (within the past 10 years)

    • Meningococcal vaccination (within the past 10 years)

  • Physical Exam: Current State of Illinois school physical examination (Sports physical forms cannot be accepted). A current physical is an exam dated less than one year from the 1st day of school

  • Dental Exam: Illinois law requires a current dental exam for 6th graders. The dental exam is to be turned in by May 15th of the current school year.  It can be dated up to 18 months of the May 15th deadline. If parents are without dental insurance or cannot afford the exam they will be required to fill out the Dental Exam Waiver form for their child.  The waiver form will fulfill the state requirement.

 

Required for Ninth Grade - 

  • Complete list of all immunizations

  • Physical Exams: Current State of Illinois school physical examination (Sports physical forms cannot be accepted); Current physical is an exam dated less than 1 year from the 1st day of school

  • Vision Exam: performed by a licensed Optometrist or Ophthalmologist (only if a student moved from another state)

  • Dental Exam: Illinois law requires a current dental exam for 9th graders. The dental exam is to be turned in by May 15th of the current school year.  It can be dated up to 18 months before the May 15th deadline. If you are without dental insurance or cannot afford the exam you will be required to fill out the Dental Exam Waiver form for your child.  The waiver form will fulfill the state requirement.

 

Required for Twelfth Grade -

  • Meningococcal Vaccine: Two doses for students entering 12th grade; if the first dose was given on or after the 16th birthday, only one dose is required
